Can You Prevent Macular Degeneration? What AREDS 2 Reveals About Protecting Your Vision

Age-related macular degeneration (AMD) is the leading cause of vision loss in adults over 50.  It affects the macula, the central part of your retina responsible for sharp, detailed vision,  making it harder to read, recognize faces, or drive safely.

The scary part? Many people don’t know they’re at risk until damage has already started.

The good news? The landmark AREDS 2 study shows us exactly how to reduce your risk. And if you do develop AMD, breakthrough treatments are now available that can slow progression and sometimes stabilize sight. At this time there is no cure for AMD. 

Understanding the Two Types of AMD

AMD comes in two forms, and knowing which one you’re at risk for matters:

Dry AMD (Atrophic)

  • Most common (85–90% of cases)
  • Caused by gradual breakdown of light-sensitive cells in the macula
  • Progresses slowly,  but can eventually lead to vision loss
  • Currently no FDA-approved treatment (but prevention is key)

Wet AMD (Neovascular)

  • Less common but more aggressive
  • Abnormal blood vessels grow under the macula and leak fluid
  • Can cause rapid vision loss if untreated
  • Now has multiple FDA-approved treatments that work remarkably well to reduce progression

What AREDS 2 Taught Us (And Why AREDS 3 Matters)

The original AREDS study (conducted in the 1990s) was groundbreaking. It showed that specific antioxidant vitamins and minerals could slow AMD progression in people with intermediate or advanced disease. 

The AREDS formula included:

  • Vitamin C
  • Vitamin E
  • Zinc
  • Copper
  • Beta-carotene

But AREDS 2 made an important discovery: 

Researchers replaced beta-carotene (which can increase lung cancer risk in smokers) with lutein and zeaxanthin. Two plant pigments found in leafy greens. They also added omega-3 fatty acids.

The result? The new formula was safer and equally effective at slowing AMD progression in people with intermediate or advanced disease. 

So is there an AREDS 3?

There is no  AREDS 3 trial at this time. An AREDS 3 trial would probably address :

  • Inflammation pathways
  • New drug therapies for early-stage disease
  • Mitochondrial role
  • Exploring other carotenoids like meso-zeaxanthin 

Confusion exists: there is a popular vitamin company that has named their newest AREDS vitamin AREDS 3. This is a brand name specific to them. 

The takeaway for you right now. 

If you’re at risk for AMD, the AREDS 2 formula is your best evidence-based nutritional defense. 

Are You at Risk for AMD? Know the Red Flags

Your risk increases if you have:

  • ✦ Age over 50 — AMD is rare before age 50 but becomes increasingly common after
  • ✦ Family history of AMD — genetics play a significant role
  • ✦ Smoking — one of the strongest modifiable risk factors
  • ✦ High blood pressure or heart disease — cardiovascular health and eye health are connected
  • ✦ Obesity — linked to increased AMD risk
  • ✦ Excessive sun exposure — UV damage accumulates over time
  • ✦ Poor diet — low in antioxidants and omega-3s
  • ✦ Sedentary lifestyle — lack of physical activity increases risk

The good news.

 Most of these are modifiable. You have more control than you think.

Prevention: What You Can Do Right Now

1. Eat for Your Eyes

The foods you eat directly impact your macula. 

Lutein and zeaxanthin (the stars of AREDS 2) are found in:

  • Dark leafy greens: spinach, kale, collard greens
  • Broccoli, Brussels sprouts, green beans
  • Eggs (especially the yolks)
  • Avocado, pistachios

Antioxidants fight cellular damage:

  • Berries (blueberries, blackberries)
  • Citrus fruits (oranges, grapefruit)
  • Bell peppers, tomatoes
  • Dark chocolate (70% cacao or higher)

Omega-3 fatty acids protect your retina:

  • Fatty fish: salmon, mackerel, sardines, trout
  • Walnuts, flaxseeds, chia seeds
  • Fish oil supplements (if you don’t eat fish)

Aim for: At least 2–3 servings of leafy greens per week, fatty fish 2–3 times per week, and a rainbow of colorful fruits and vegetables daily.

2. Consider the AREDS 2 Supplement (If Recommended)

If you have intermediate AMD or a strong family history, your eye doctor may recommend an AREDS 2 supplement. 

Talk to your optometrist about whether this is right for you. Not everyone needs it, but for those at risk, it can make a measurable difference.

Popular AREDS 2 formulas include:

  • Other pharmacy brands (ask your eye doctor which they recommend)

3. Quit Smoking (If You Smoke)

Smoking is one of the strongest modifiable risk factors for AMD. If you smoke, quitting is the single most impactful thing you can do for your eye health.

Resources:

  • Talk to your doctor about nicotine replacement or prescription medications
  • Call 1-800-QUIT-NOW for free support
  • Ask about local smoking cessation programs in Dana Point

4. Protect Your Eyes from UV Damage

UV exposure accumulates over a lifetime and increases AMD risk. 

What to do:

  • Wear 100% UV-blocking sunglasses every time you’re outside (even on cloudy days)
  • Choose sunglasses that block both UVA and UVB rays
  • Wear a wide-brimmed hat for extra protection
  • Avoid peak sun hours (10 AM–4 PM) when possible

5. Manage Your Blood Pressure and Heart Health

Cardiovascular health and eye health are deeply connected. 

  • Get your blood pressure checked regularly
  • Exercise at least 150 minutes per week (brisk walking counts!)
  • Maintain a healthy weight
  • Manage stress
  • Follow your doctor’s recommendations for heart health medications

6. Get Regular Eye Exams

This is critical. Early AMD often has no symptoms. By the time you notice vision changes, significant damage may have already occurred.

How often should you be screened?

  • Age 40–50: Every 2–4 years (or as recommended by your eye doctor)
  • Age 50–60: Every 1–2 years
  • Age 60+: Every year (or more frequently if you have risk factors)

New Treatments: What’s Available Now

If you do develop AMD, the good news is that treatment options have expanded dramatically.

For Wet AMD: Anti-VEGF Injections

If abnormal blood vessels develop under your macula, anti-VEGF (anti-vascular endothelial growth factor) injections are the gold standard treatment. 

These medications stop the growth of abnormal blood vessels and can preserve or even improve vision.

Available options include:

  • Avastin (bevacizumab)
  • Lucentis (ranibizumab)
  • Eylea (aflibercept)
  • Beovu (brolucizumab)
  • Faricimab (a newer option with longer-lasting effects) 

How they work: Injected directly into the eye (yes, it sounds scary, but it’s quick and relatively painless). Most patients receive injections every 4–12 weeks, depending on the medication and how their eye responds.

Results: When caught early and treated promptly, anti-VEGF therapy can slow/halt vision loss and help many patients maintain or regain functional vision.

For Dry AMD: Emerging Options

Dry AMD has been tougher to treat, but that’s changing.

Current approaches:

  • AREDS 2 supplements — slow progression in intermediate/advanced disease
  • Nutritional optimization — maximize lutein, zeaxanthin, and omega-3s
  • Lifestyle modifications — smoking cessation, UV protection, exercise
  • Low-vision aids — magnifiers, specialized glasses, and software to maximize remaining vision

New approach:

Low-Level Light Therapy (LLLT) also called red light therapy: What You Should Know

You may have heard about low-level light therapy (LLLT), also called photobiomodulation, as a AMD treatment.

What is it?

LLLT uses specific wavelengths of light (typically red or near-infrared) to stimulate cellular energy production in the retina. The theory is that this can help damaged retinal cells function better and slow AMD progression.

Current status:

  • FDA-approved in November 2024 for the treatment of Dry AMD only 
  • first device of this kind is the Valeda Light Delivery System

Should you try it? Talk to your eye doctor to see if you are a good candidate. 

On the horizon:

  • Gene therapy — early research shows promise for treating certain forms of AMD 
  • New drug therapies — currently in clinical trials

Your AMD Prevention Action Plan

Starting today:
